Pre-orientation health history/immunization form

You will need to send a hard copy of your immunizations that can be attained from your health care provider, high school, or previous college. It is not necessary to send a hard copy of your health history if you've completed the online form.

Health history and immunization requirements must be fulfilled before you may register for an orientation session.

The pre-particpation sports physical exam (PDF): New NCAA Intercollegiate student-athletes (not club or intramurals) are required to have this form completed by their home health care provider before tryouts and practice.  Please fax completed form to Health Services (315.312.5409)
