Pre-orientation health history/immunization form

The Health history and immunization form (PDF) is available for you to download. Directions are contained within the form as is the address for mailing the form to Mary Walker Health Center, SUNY Oswego, Oswego, NY 13126. 

Students have the option to complete their health history onlineFollow the link from the new student menu page on MyOswego. Once your email account is activated you may follow the link to health requirements. Follow link for medicat connect. Log in using your LakerNet ID and password. Click link on left of screen for "Medical History." Complete the questions and submit. This goes directly to your secure electronic medical record. There is also a direct link on our main web page.

You will need to send a hard copy of your immunizations that can be attained from your health care provider, high school or previous college. It is not necessary to send a hard copy of your health history if you've completed the online form. 

Health history and immunization requirements must be fulfilled before you may register for an orientation session.

The pre-particpation sports physical exam (PDF): New NCAA Intercollegiate student-athletes (not club or intramurals) are required to have this form completed by their home health care provider before tryouts and practice.  Please send this with the health history (online OR hard copy) and immunization form (hard copy).
