Option Transfer Period, Opt Out Program Enrollment and Tax Change Period Reminder

This is a reminder that the Option Transfer Period, Opt Out Program Enrollment, and Tax Change Period will end on Thursday, December 31, 2020. Please return your signed and completed enrollment forms to the Office of Human Resources by one of the following methods:

  • Submit forms by drop off in the mail slot on the door of 201 Culkin Hall; OR
  • Send through inter-campus mail to 201 Culkin Hall; OR
  • Submit electronically to melissa.newman@oswego.edu

If submitting electronically, please do not enter date of birth or any social security numbers on the form to protect your confidentiality. If there are any issues with the forms, HR will reach out to directly to the employee.

Option Transfer

If you would like to change your health insurance option for 2021, you must complete a NYS Health Insurance Transaction Form located here and return to Human Resources Office by December 31, 2020. 

Please see the 2021 Health Insurance Choices booklet which outlines the benefits of each plan and the 2021 ratified rates and 2021 unratified rates for each plan.

Opt Out Program

If you are currently enrolled in a health insurance plan and are interested in transferring to the 2021 Opt Out Program, you must complete both the PS-404 and PS-409 and return them to the office of Human Resources Office by December 31, 2020.

Tax change period

Enrollees in the NYS Health Insurance Program (NYSHIP) may elect to patriciate or decline participation in the Pre-Tax Contribution Program (PTCP) for the upcoming plan year. If you wish to change your PTCP selection for 2021 please complete the following form and return to HR by December 31, 2020.
