If you are watching a recorded video to satisfy a requirement for Academic Probation or another requirement, please complete the assessment form that corresponds with the respective video. Your advisor or instructor will be notified of your participation as long as an accurate email address is indicated on the assessment form. These forms will become available again in August 2022.
