EOP Summer Program Checklist

Please read and complete the following steps.  Please note the different due dates for each step.  

All steps must be completed before you can attend the summer program. 

Step 1: Please complete all the parts of the process located at the New Student Orientation page. These steps must be completed before we can sign you up for classes and before you can start the Summer Program. The sooner you complete step 1, the sooner your EOP Advisor will be able to register you for classes. 

Step 2: Please complete and submit the EOP Summer Program Registration form by Friday, June 17th .

Step 3:  Review and sign the EOP Summer Program Policies and Participation Agreement AND Curriculum Overview.  If you are under the age of 18 years old, you will need your parent or guardian to sign the forms.  Please return the signed form via email to eop@oswego.edu or fax: 315-312-5676 by Friday, June 17th.

Step 4:  Summer Food Service Program (SFSP).  One of the ways we supplement the food costs associated with running our Summer Educational Opportunity Program is through the Summer Food Service Program grant offered by the New York State Education Department. The grant requires that we have the attached form completed and signed by you for each individual student. Please return completed and signed Summer Food Service Program form via email to eop@oswego.edu or fax: 315-312-5676 by Monday, June 27th
