EOP Summer Program Registration and Enrollment Checklist

All steps must be completed by the due dates in order to  attend the summer program. 

Step 1:  EOP Summer Program Registration Form:  Please complete and submit the EOP Summer Program Registration Form (2023 Form Coming Soon) by TBA.

Step 2: EOP Summer Program Policies and Participation Agreement:  Review and sign the EOP Summer Program Policies and Participation Agreement (2023 Agreement Coming Soon).   If you are under the age of 18 years old, you will need your parent or guardian to sign the forms. Please return the signed form via email to eop@oswego.edu or fax: 315-312-5676 by TBA.

Step 3:  Curriculum Overview:  Review and Sign the Curriculum Overview.  (2023 Form Coming Soon) If you are under the age of 18 years old, you will need your parent or guardian to sign the forms.  Please return the signed form via email to eop@oswego.edu or fax: 315-312-5676 by TBA.

Step 4:  Summer Food Service Program Form:  Please return completed and signed Summer Food Service Program Form (2023 SFSP Form Coming Soon) via email to eop@oswego.edu or fax: 315-312-5676 by TBA.  One of the ways we supplement the food costs associated with running our Summer Educational Opportunity Program is through the Summer Food Service Program grant offered by the New York State Education Department. The grant requires that we have the attached form completed and signed by you for each individual student.
