Laptop Payment Plan Agreement

            The Laptop Payment Plan Agreement needs to be signed prior to or upon pickup of Laptops. To expedite the pickup process, you may mail in or drop off the completed signature pages before move in day.  A co-signor must be someone who is not a spouse, but is over 21 years of age and is accepting responsibility as a joint debtor to Auxiliary Services. The College BookStore requires the signature pages only.  Feel free to print and keep a copy for your own records.

              Please complete the Laptop Payment Plan Agreement as follows (The Customer Signature is the student signature)

             The Assigned Equipment information will be completed by College BookStore  Staff at  time of  pickup.
