The Health History and Immunization Form (PDF) is available for you to download. Directions are contained within the form as is the address for mailing the form to Mary Walker Health Center, SUNY Oswego, Oswego, NY 13126. This is a requirement that must be fulfilled BEFORE you may REGISTER for an Orientation Session.

 

The Pre-Particpation Sports Physical Exam (PDF) New NCAA Intercollegiate student athletes (not club or intramurals) are required to have this form completed by their home health care provider BEFORE tryouts and practice.  Please send this with the Health History and Immunization Form.
