Pre-Orientation Health History & Immunization Form
The Health History and Immunization Form (PDF) is available for you to download. Directions are contained within the form as is the address for mailing the form to Mary Walker Health Center, SUNY Oswego, Oswego, NY 13126. Students have the choice to complete their Health History online. Follow the link from the new student menu page. Once your email account is activated you may follow the link . There is also a direct link on our main web page. Using your Laker net ID and password you may complete the form which goes directly to your secure electronic medical record. You will need to send a hard copy of your immunizations that can be attained from your health care provider, high school, or previous college. It is not necessary to send a hard copy of your Health History if you've completed the online form.
Health History and Immunization requirements must be fulfilled BEFORE you may REGISTER for an Orientation Session.
The Pre-Particpation Sports Physical Exam (PDF) New NCAA Intercollegiate student athletes (not club or intramurals) are required to have this form completed by their home health care provider BEFORE tryouts and practice. Please send this with the Health History (online OR hard copy) and Immunization Form (hard copy).
