The test scoring sheets may be purchased through Central Stores, the item number is 5500.

If you have any questions on the following procedures, please contact us before you set up your exam. There needs to be two sheets provided by you as the instructor.

A) The first sheet you will need to provide is the control sheet. This sheet provides us the specific information we need to run your exam properly. Please follow the procedures carefully. Wrong information on your control sheet may cause a delay not only for you but your colleagues as well!

B) The next sheet is the answer key. This sheet should have only the answers to the exam on it.

Control sheet Information:

NAME AREA

1) Fill in instructors last name

2) In the last 3 positions of the name field please insert the Course Dept. (ex. ART)

BIRTH DATE AREA

1) Fill in today's date

IDENTIFICATION NUMBER AREA

COLUMN A:

This column should be filled in with 0, 1, or 2 depending on which of the following options you want:

  • 0 = Test scoring and item analysis (standard)
  • 1 = Questionnaire and analysis
  • 2 = Test scoring analysis with Id number sort file saved for electronic transfer to instructor for downloading purposes. (This includes the students ID number, their total score on exam and their Z SCORE)**

COLUMN B:

This column gives the point value assigned to each item. If a number between 1 and 9 is marked, each item will have that value. If 0 is marked, then you may assign a different value to each item by filling in the answer part of this sheet with value you want assigned to each item. Please remember that when a 0 is filled in all answers are automatically assigned a value of 1. By filling in A the value would be 2 points each, B the value would be 3 points etc., so by leaving a bubble unfilled a value of 1 point is assigned. The maximum point value using this option would be 6.

COLUMN C:

This column indicates where to get the names for the listings:

  • 0 = Names taken from test sheet
  • 1 = Registration records searched by ID number to find name

COLUMN D:

This column should always be filled in with 0

COLUMNS E, F and G:

  • If column A = 0 or 1 ----- blank.
  • If column A = 2 ------ Multi-mark these three columns with a 0,1 and 2.

COLUMNS H, I AND J:

These columns indicate the number of items on the test.H has to be filled in with a zero if your items are less than 100, ex. 050 = 50 items.If you fail to do this your exam will BE DELAYED in processing as will any other exams that are being scored.

PLEASE NOTE: Following are the listings you will receive:

1.) Your answer key as read by the scanner

2.) Listing of students sheets in the order in which they were scanned. A dot represents a correct response, a number represents an incorrect response and which wrong answer the student gave.

3.) Listing of test score results in the following order:

a) ID number b) Highest to lowest scores c) Alphabetical

4.) At the end of this run you will receive an Item Analysis. This gives you information on each question you asked. An asterisk above the number indicates what you put on your key as a correct answer. This listing allows you to see if any of the questions you asked were "bad" questions.
