    Freshmen Forms

Students applying as Freshmen are sometimes asked to submit additional forms.  These forms are not required of all students.  Please contact the Office of Admissions at 315.312.2250 or admiss@oswego.edu if you have questions regarding which forms are required for your application.

• Oswego's Supplement to the SUNY application
  (online...completed online & submitted via email) or (printable... to be completed by hand and mailed)
• Oswego's Supplement to The Common Application
  If filing using the paper application please submit either version. (online...completed online & submitted via email) or print and mail the paper Supplement to The Common Application.
• Major/Area of Concentration Sheet
  (online...completed online & submitted via email) or (printable...to be completed by hand and mailed)
• EOP Personal Data Form (Freshman form, complete only if you were directed to by the Admissions Office)
  For Spring 2010 applicants.  Please download (463 KB pdf.), complete and mail.
• EOP Personal Data Form (Freshman form, complete only if you were directed to by the Admissions Office)
  For Fall 2010 or Spring 2011 applicants.  Please download(238 KB pdf.), complete and mail.
