Accident and Sickness Insurance is required of all FT students (Undergraduate 12hrs - Graduate 9hrs) unless they submit a waiver showing they have medical coverage. To get the Insurance Waiver form, click on the link below. A new window will open up just with the Insurance Waiver PDF document.

Then fill out the form and send it to Auxiliary Services, 507 Culkin Hall, Oswego State University, Oswego, NY 13126

The 2008/2009 waiver is not yet available. We will enable the link as soon as it is available.
