Forms, information for prospective student-athletes.
All Students, Undergraduate and Graduate, must apply to graduate in order to receive their degree. Applying on time will avoid possible graduation problems and ensure your name is printed in the commencement program. Apply online on the Student Records tab in myOswego:
Sign in to myOswego (http://myoswego.oswego.edu)
Click on the "Student Records" tab
Click on "Apply for Graduation"
Follow the on-screen instructions.
Undergraduate students can make changes to their majors/minors by visiting the appropriate Academic Department and submitting a Declaration of Major/Minor Form to 302 Culkin Hall.
Undergraduate students must meet with their advisor for each of their major(s) and minor(s) and complete a Senior Check Form to be submitted to the Registrar's Graduation Area, 307 Culkin Hall, by the appropriate deadline. The Senior Check Form is mandatory for graduation. Beginning with December 2013 degree candidates, Senior Check Forms can be Downloaded from Degree Works or from our Forms page.

Dates to File Online: April 1, 2012 to October 1, 2012*
Deadline to return Senior Check Form: December 1, 2012
May Commencement ceremonies will be held in the Campus Center
August Graduation - (Date only, no ceremony**) August 16, 2013
Dates to File Online: April 1, 2012 to February 1, 2013
Deadline to return Senior Check Form: April 1, 2013
December Graduation - December 14, 2013
Dates to File Online: Jan 1, 2013 to April 1, 2013
Deadline to return Senior Check Form: May 1, 2013
December Commencement ceremonies will be held in Campus Center
*Do not file for May if you have coursework to complete during the summer.
**Students who file for August Graduation are automatically included in and invited to participate in the May Ceremony.
Please note: If you miss the above deadline dates to file online, or if you cannot apply to graduate online, you will need to contact the Graduation Area of the Registrar's Office for help.
