Pass-Fail Option Form

Important Notes

1. Students of junior or senior standing, with at least a 2.00 cumulative grade point average, are permitted to register for a maximum of one Pass-Fail option course per grading period, including summer sessions. Non-degree students are also allowed to opt for Pass-Fail.
2. Pass-Fail courses are limited to those courses taken to fulfill elective courses only. A course used to satisfy general education, major, minor, concentration, or cognate requirements may not be taken on a Pass-Fail basis. If you have any questions whether a course may be taken Pass-Fail, please contact your advisor. (See catalog under Curriculum Information)

 By signing this form I am choosing to take the designated course on a pass-fail grading basis. I acknowledge that I have read and understand the requirements and consequences of taking a course on a pass-fail grading basis as detailed in the college’s undergraduate catalog found in the online College Catalog.  I understand that the Pass-Fail option is NOT available for courses needed for a major or concentration, including professional education courses and cognate requirements, as well as courses needed for either a minor requirement or General Education requirement.

Pass-Fail option requests for semester (or quarter) courses are considered until the add deadline for semester (or quarter) courses.
