Please read all sections of the form carefully as it includes registration information, the EOP Summer Program Policies and Participation Agreement, and the Curriculum Overview. If you are under the age of 18 years old (prior to July 1, 2024), you will need your parent or guardian to sign the forms.
Summer Food Service Program Form
Please return completed and signed via email to [email protected]. One of the ways we supplement the food costs associated with running our Summer Educational Opportunity Program is through the Summer Food Service Program grant offered by the New York State Education Department. The grant requires that we have this form completed and signed for each individual student.
