Forms, information for prospective student-athletes.
Are you interested in being a Mentor-Scholar during the 2012-2013 academic year? Go through the following checklist before you sign up:
1) Am I available from 2pm to 3:45pm on either Mondays/Wednesdays or Tuesday/Thursdays?
2) Am I interested in committing a minimum of 40 hours of my time to mentoring an at-risk Oswego Middle School student?
3) Am I able to commit to working with a mentee for an entire academic year (Fall 2012 and Spring 2013)?
If you meet all of these criteria, please apply for our program by downloading a copy of the Mentor-Scholar Application 2012-2013. If you complete the application electronically, email your finished application to Project Coordinator, Patricia Waters. You may also complete the application by hand and bring your finished application to Patricia at Campus Center 145L (located in the Compass). Once your application is received, we will put you on the path to joining Mentor-Scholar.
