II.Credit Hours  

Designate credit hours appropriate to the course content  

III.Course Title

Assign a course title that briefly and adequately identifies the course content  

IV.Course Type
Specify if the course is new or an update to an existing course.




Write a concise course description for the college catalog (approximately 30-40 words in length).

VI.Extended Course Description 

Write a second description that adequately informs prospective faculty of the course goals and content.

Identify prerequisite courses, class standing, and experiences. Indicate the criteria that will be used to waive such prerequisites.

VIII.Frequency of Course Offering
Indicate all that apply: 
Fl - every Fall
Sp - every Spring
SS - every Summer
Fl - irregular basis

IX.Justification for the Course 

Justify the need for the course in a particular program of study. Include such details as: Targeted student population, rationale for class size, rationale for elective or required status, and why the course is offered at a particular level.

List the course objectives in behavioral terms (i.e. list what the student is expected to do as a result of this course)

List topics and activities of the course. Ensure the topics and activities adequately address goals and objectives stated above.

Indicate the methods of instruction. Ensure consistency among methods, goals, objectives, topics/activities, and requirements.

Indicate expectations for students in order to complete the course successfully. Ensure consistency among requirements, objectives, methods of instruction, and evaluation.(Please number your requirements.)


 

XIV.Means of Evaluation

Indicate means of evaluating student performance. Ensure consistency between requirements and evaluation methods.

XVI.Bibliography:
