Course Submission Form

Fill in every field of the form

Before submitting a course, please read this page

A course proposal  template that can be used at the department level is here.  All course proposals must be submitted via the webform.  Please keep in mind that most formatting, particularly indentations in outlines, are not kept when submitted via the webform, so simple text is the best.

Note: Due to increasing number of "spam" going through the webform, course submission is blocked from non-campus computers.

(position):

Assign a course prefix and number appropriate to the course content.  Each 100 level should represent difficulty or course (eg: 100 for freshman level; 200 for sophomore level; 300 for junior level; and 400 for senior level).
Dedicated numbers include:  399 or 499 for Independent Studies; 498 for Internships; 497 for Capstone courses (with some exceptions).
Do not include letters within course number (eg: 395a).
Do not use a course number for new courses that has already been used. Check with the Registrar's office if you are not sure which numbers are available for your program.

Course Type
Specify if the course is new or an update to an existing course.

Assign a course title that briefly and adequately identifies the course content.

Write a concise course description (50 words maximum) for the college catalog, including the course goals and/or content.

Identify prerequisite courses and/or experiences. Indicate the criteria that will be used to waive such prerequisites. Courses at the 300 and 400 level should have prerequisites. Avoid cascading prerequisites. Instead include as a prerequisite, only the last course the student would be expected to take in a line of sequential courses.

Frequency of Course Offering
Indicate all that apply:
Fl - every Fall
Sp - every Spring
SS - every Summer
Fl - irregular basis

Designate credit hours appropriate to the course content.

Justification for the Course
Justify the need for the course in a particular program of study. Include such details as: Targeted student population, rationale for class size, rationale for elective or required status, and why the course is offered at a particular level.

Course Objectives  List the course objectives in behavioral terms (i.e. list what the student is expected to do as a result of this course)

Please give an expanded description of the course content. This might include an outline, bulleted lists or paragraphs, or a prose description. UCC will not enforce a specific format for the Course Description. However, most course outlines contain the following:  Justification or Rationale for offering the course; Course Content; Methods of Instruction; Course Requirements; and Means of Evaluation.

If necessary, follow the expectations of your departments and/or programs particularly as they relate to other campus requirements, or the accreditation process (for example: General Education, the Schools of Education or Business, or the Art Department).

Indicate the digital, print, and/or computer technology resources (for faculty or students)  that you intend to use, or are currently using, when offering this course.

If you will need additional resources, please contact your subject librarian in Penfield Library, and if appropriate, the Campus Technology Services, Chief Technology Officer.

Provide a bibliography that reflects course content, written in a consistent style (such as APA, MLA, etc.).

Make any special comments you feel may be relevant to course approval. If this course is a revised or updated course, indicate where you have made changes and the reasons for these changes.
